Holland Dermatology for your dermatological needs. It is our

the confidence and trust that you have placed in us.
While we welcome patients of all ages to our practice, it is
necessary that any patient under the age of 18 be accompanied by
a parent/ legal guardian or present a signed
permission slip
in order to receive treatment of any kind.
Please take the time to fill out the appropriate
forms now available on
our website and bring them along with you to your
appointment. This will help expedite your check-in process.
The information that follows is designed to answer many
of the questions most patients have regarding our fees and
payments.
We participate with
Medicare.
We will bill you for any deductibles. For all
other insurance
carriers, please check with us to see if we are contracted
with them. Co-pays are expected at the time of service. Please
bring proof of your insurance to all of your visits.
