While we welcome patients of all ages to our practice, it is necessary that any patient under the age of 18 be accompanied by a parent/legal guardian at their initial visit. Follow-up visits may be unattended if the parent has presented a signed permission slip for the treatment of a minor.
New patients (parent/legal guardian if a minor) must present a valid government photo ID as proof of identity. This is required in order to prevent the occurrence of insurance and/or payment card fraud. Photo ID may be requested upon check-in at any time. Please be sure to have this ID with you in order to keep your appointment.

To expedite the check-in process, we ask that all new patients please take the time to fill out the appropriate forms and bring them along with you to your appointment.

If you provide us with current insurance details, we will submit a claim on your behalf. Copays are due at time of service. We participate with most plans offered by major insurance companies in West Michigan, however, narrow network plans are becoming more commonplace. These plans limit which providers are in the network.
Please note that it is your responsibility to confirm with your insurance company that we participate with your plan and that no referral or authorization is required from your PCP prior to your visit. The easiest way to find this out is by calling the number on the back of your insurance card and asking them if they participate with Dr. Barbara Drozdowski.
